    • Completed applications must be approved by Superintendents prior to submission.
    • For the sake of this Award, "Local Food" is defined as food that is grown, raised, and produced in Georgia or a state touching Georgia. "Produced" refers to products made locally from primarily locally grown or raised ingredients.

  • There is an additional award category, the Outstanding Award, which recognizes the district/LEA with the outstanding farm to school program in the 2015-2016 school year.

  • CRITERIA 1: Feature locally grown items in the school cafeteria, during breakfast, and/or lunch.

  • Please share at least ONE of the following documents.

    • List of all locally grown items served
    • Distributor order or delivery form indicating local items
    • School menu with featured local food notated
    • Photograph of school meal tray * with local items noted
  • CRITERIA 2: Students participate in taste tests that feature locally grown food.

  • Please share at least ONE of the following documents.

    • List of all taste tested items
    • Name of farm which taste tested items were sourced
    • Taste test survey results
    • Quotes of students opinions about the taste test(s)
    • Photograph of taste test

     

  • CRITERIA 3: Students interact with a farmer through field trips to farms that grow food and/or through farmer visits to the school.

  • Please share at least ONE of the following documents.

    • List of farmers with whom students interacted, including their farm name
    • List of activities done with a farm or farmer visit
    • Photograph of a farm field trip or farmer visit
  • CRITERIA 4: Farm to school promotions are done throughout the school environment

  • Please share at least ONE of the following documents.

    • Sample of farm to school promotional materials used
    • Photograph of posted materials
  • CRITERIA 5: Teachers integrate farm to school into standards-based curriculum.

  • Please share at least ONE of the following documents.

    • List of lesson names integrating farm to school
    • List of Georgia Performance/Common Core Standards addressed through farm to school-integrated lessons
    • Photograph of farm to school-integrated lesson being taught
    • Student work from a farm to school-integrated lesson
  • CRITERIA 6: Schools have edible gardens.

  • *Edible gardens mean growing at least one edible plant at school. Edible gardens can include in ground or raised beds, a greenhouse with edible plants, and pots of herbs or other edible plants. Examples:

    • Five raised beds in the same area at a school is ONE garden
    • Three teachers each with their own set of indoor edible plants is THREE gardens
  • Please share at least ONE of the following documents.

    • List of all schools with edible gardens
    • Photograph of garden
    • Photograph of children engaged in educational activities in the garden
    • Schedule of class time spent in the garden
    • List of educational activities taught in the garden
    • School garden maintenance schedule
    • School garden safety procedures
    • School nutrition menu featuring school garden produce
  • CRITERIA 7: Students participate in hands-on cooking and food activities with chefs, teachers, parents, school nutrition staff, or other leaders.

  • Please share at least ONE of the following documents.

    • List of cooking and food activities that occurred in your district/LEA
    • Photograph of students preparing food
    • Recipe students prepared
    • List of community volunteers (i.e. chefs, parents) who helped lead food activities
  • CRITERIA 8: Parents and/or community members are involved in the farm to school program.

  • Please share at least ONE of the following documents.

    • List of activities in which parents or community members were involved
    • Parent/community outreach material (i.e.: flyer, newsletter article, etc)
    • Photograph of parent/community involvement
    • List of parents and community members who were involved in farm to school activities
  • CRITERIA 9. School staff (i.e.: teachers, school nutrition staff, administration, etc.) participate in farm to school professional development training.

  • *Please note that the professional development training must be school-wide or district-wide to count. Select staff members attending a conference that includes farm to school content does NOT count as school-wide or district-wide professional development.

  • Please share at least ONE of the following documents.

    • List of trainings that occurred, with farm to school relevant pieces notated
    • Training agenda
    • Material provided at training
    • Photograph of training
    • List of all school staff who participated in training
  • CRITERIA 10: Farm to school language is adopted into an LEA-wide policy or procedure (i.e.: Wellness Policy).

  • Please upload the following document.

    • Copy of policy or procedure with farm to school language notated
  • I attest that my District/LEA Superintendent has granted Georgia Organics and the Georgia Departments of Education, Agriculture, and Public Health permission to use the attached images in email communications, websites, and print materials promoting the practice of farm to school. By typing my name below, I attest that I have on file, and can produce if requested, release forms for all minor children who can be identified in the attached images.
